Who the list actually includes
The category was announced on 8 December 2025, part of a federal package aimed at boosting Canada’s supply of doctors. Ask which doctor occupations qualify for the physicians category and the answer is short: general practitioners and family physicians, specialists in surgery, and specialists in clinical and laboratory medicine. Three lines on a list, nothing more. Radiologists, psychiatrists and most other specialities aren’t on it, so “physician” in the category’s name is narrower than it sounds.
A record-low cut-off, and what it doesn’t mean
The first round under this category ran on 19 February 2026: 391 invitations at a Comprehensive Ranking System cut-off of 169 — the lowest since the pandemic era. Read that number on its own and it looks like Express Entry suddenly got easy. It didn’t. A cut-off that low happens when almost everyone eligible to apply already clears it, because the pool of candidates who meet the “Canadian experience, in one of three occupations” bar is tiny. The second round, on 24 June 2026, invited 271 people at CRS 223 — still well below what a Canadian Experience Class or Provincial Nominee draw asks for, and still drawing from that same small, specific group.
Why this doesn’t reach South Africa yet
The experience requirement is explicit: it has to be gained in Canada, not abroad. A distinguished surgical career built entirely in Gauteng or the Western Cape counts for nothing toward this particular category, however senior the role or however long the CV. That’s the honest answer to whether the physicians category helps a doctor still practising in South Africa — not yet, and not without first finding a way to work in Canada in one of those three roles for a year.
There is a related but separate route worth knowing about. The same December 2025 package gave provinces extra federal nomination room specifically for doctors who already have a Canadian job offer, plus faster, 14-day work permit processing once nominated. That’s a provincial nomination pathway, not this Express Entry category, and it runs on its own rules and timeline. The two shouldn’t be confused with each other when you’re trying to work out which door, if any, currently applies to your situation.
The realistic reading, and the point of this physicians with Canadian work experience category briefing, is that a locum placement, a fellowship, or any legitimate route into a year of Canadian practice in one of the three listed roles is what turns this from an interesting headline into something usable. Until then, it sits alongside the other Canadian-experience-only categories as proof that Express Entry increasingly rewards people already inside the country, not people trying to get in.
