It isn’t the physician’s calendar that gets crowded in that window so much as everything around it, and the timing is worth planning against.
Your GP isn’t an option here
Short answer: can I use my own doctor instead of a panel physician? No. Only an IRCC-approved panel physician can perform the immigration medical exam a PR application requires. Your regular GP, however good, can’t do this one — the exam has to go through someone specifically approved for the role, with results transmitted directly to IRCC rather than handed to you as a letter.

IRCC requires the medical exam for all family members connected to a PR application, even those not coming with you. This surprises almost everyone, and it’s worth raising with a spouse or adult child staying behind in South Africa before the instructions land rather than after.
Booking around the seasonal crush
If your application timing touches the March–April window when SAPS clearance requests and visa renewals both spike, book earlier rather than later in that stretch. Nothing about the exam itself changes with the season, but everything adjacent to it — courier turnaround, appointment availability, the general backlog — gets slower.
